Confidential Information and Wishes Packet
Purpose
To provide information for the Department and
family members during a line of duty death, off duty death, or serious injury of
a department employee
General Guidelines
Employee Confidential Information and Wishes
Packets will be given to each employee upon hiring to be completed in case
of a line of duty death or serious injury.
The packets will be kept under lock and key and at no time be viewed
other than in the event of the employee’s line of duty death, off duty death, or
serious injury.
The packets will be available to be modified by
the employee and only the employee if so necessary.
Once the initial packet is completed or an existing packet
is modified, the packet will be sealed and the employee shall sign his/her name
in permanent market across the seal to ensure that the integrity of the envelope
is maintained.
The information entered is designed to assist
the employee’s family and the department in providing for the wishes of the
employee in the event of a line of duty death, off duty death, or serious
injury.
